Hallam24 Healthcare has a range of homecare services to suit all needs and budgets regardless of complexity. Our Hospital to home services can assist individuals leaving hospital with all aspects of recovery by arranging short daily visits to 24-hour specialist practitioner visits. We understand fully the need to work with each individual and their family, to ensure that the right level of care is provided through each stage of the process of leaving hospital. We also liaise closely with the GP, hospital consultant and community health team of individuals coming home from hospital, to ensure continuity of care.
